Step three:

In a new layer draw a black rectangle as seen marqueed selected on the picture guide.

Add the following layer styles: Gradient Overlay.



Step four:

Make a marquee selection on the center of both shapes. Fill selection with black in a new layer.

Add the following layer styles: Inner Shadow, Gradient Overlay.
